ORDER BY DEC 5th to get clothing before christmas!!

• To get your order by Christmas, it must be placed no later than Friday, December 5^th.  If you want your order for Christmas, I do not recommend waiting for the online store to open.  As in the past, there are a few custom made items and they are not guaranteed for Christmas delivery.
• To reduce shipping costs, orders will be delivered through your Team Manager.  It is your responsibility to arrange pick-up with your Team Manager.
• There are 3 order forms.  1) Main Order Form which is 2 pages, 2) Player Package 1, and 3) Player Package 2.  I recommend that if you are personalizing items that your use one form per player for the player packages.  The main order form has a column on the right side for you to note quantities’ and personalization.  Use your best judgment as far as breaking out the main order form per player.  If it looks confusing to you, it probably will be confusing to person processing your order.
• There is a Payment Information Form that must be completed with each order.
• Orders may be faxed, e-mailed or mailed.

• Sweatshirts are available for sizing purposes and will be made available at the both arenas.  Please stay tuned for dates and times.
• On the Main Order Form, if a shirt is listed but a color choice is not, it is available in black only.  The Association Decal listed on page 2 is window decal for your vehicle.  Please disregard the “Late Fee” and the comment regarding “No Credit Cards” as this is an error and does not apply. 
• Please keep a copy of your order for your records.
